If you have the same issue then the solution is to update your graphics drivers. If you are 100% positive that your drivers are up to date, then you do not have the same issue.

It's fine if you have the old launcher. The new launcher isn't very good, even for vanilla. You won't be able to connect to old servers with it.

Looks like a problem with dimension 6, if that world ever worked correctly you could try restoring a back up of it. Alternative solutions involve using an NBT editor to move your player back to the overworld and then just avoiding that dimension.

Difficult to know for sure, there are a few things going on in this log which could be contributing. Firstly I would make sure that you don't have an antivirus product that could be scanning your game directory while playing, or make sure there isn't a problem with your hdd - the log is showing...

All of those crashes are inventory tweaks related, try disabling that and see if it helps

This is not an FTB pack, also there is no reliable way to run plugins on Forge servers, existing attempts to make this work introduce various random bugs and crashes.
