Sounds like you got a cave world in that last one. Also you might be falling foul of (what I think is) a bug in Ultimate Single Player. See my thread here, it has a workaround...

One thing to watch out for is that even if you disable Age Books and the Book Binder, but don't disable the writing desk, players can still create ages using the writing desk. If they place a normal vanilla book in the left slot of the writing desk (and provide ink and paper in the other slots)...

According to the modpack pages, Mindcrack is using v1.4.7v3. They have Mindcrack 8.2.0 in testing, but I don't see a portal gun update listed: http://forum.feed-the-beast.com/threads/mindcrack.4666/#post-157165 That's a bit odd, as the Direwolf20 pack did get the portal gun update... :/

Mystcraft is great for making a server more interesting, but the main problem with Mystcraft in a multiplayer environment is that generating chunks causes lag (from all the disk writing). So allowing players to just create ages as they see fit will cause issues for almost any server. Cave worlds...

It sounds correct, but as I said I can't test right now and I haven't blocked picking up blocks so I don't know for sure. I've only disabled the moon portals' grab blocks feature - you might want to check that out too if you haven't completely disabled moon portals altogether.
