An item has no valid destination and has become jammed in the relay. Slonk a chest down at the end of the tube system and the relay will fix itself as whatever is jamming it should flow to the chest.

srsly... don't use BC item pipes on that kind of thing :) Red power tubes would work wonders. Railcraft item loaders and unloaders would also work. Applied energistics importers and exporters would also work. I don't know if golems would work but I suspect they would.
