Davy trips and nearly falls off. He's holding onto the cliff edge with one hand, while Julietta still has her mouth on his extremity, and is biting it so she doesn't fall. This is causing Davy unimaginable pain.
Davy can't take the pain much longer of her teeth biting on to such a sensitive organ, so he either has to endure the pain and climb back up to the top or let Julietta fall, thus saving his extremity.
