This is a known bug. There is a reason that certain mobs can be blacklisted in the config of MFR. If you're using 2.1.6 I think you'll find that certain Mobs are already blacklisted.

Did you lose all your lives? It means game over. You can "cheat" and recover from the AromaBackup folder.

It doesn't detect the items in the inventory. It detects the actual crafting of them. If you use AE to craft them it won't pick up. You'll need to manually craft it. From either the AE Crafting Terminal or normal crafting table.

Just a note, the activation crystal gets bound on first use (or possibly right-click) so it is possible to craft some and on-sell them to others.

If you're in SSP you can get the OP Book and force the completion of quests (Withhold your cheating ways :D) I believe its /hqm edit You may not have to rebuild the machines. just break and re-place them (or wrench them)

Have you tried deleting everything out of the Agrarian Skies folder except the save? And try again.

A bit more information would be helpful. Did you run out of lives? It is hardcore so your world would delete when you do.
