It was the corrupt player.dat file in the HardCoreQuest folder inside the world folder. It needs deleting so a new one can be generated, this does reset quests for everyone though and effectively resets the mod. Its been reported to the HQM bug tracker, hopefully its just one of those rare odd bugs you tend to get that only ever happens once.

Hitting 30+ players now, only 20 slots left!

And maybe an Applied Energistics system with 16M storage... but only if you really feel like it.
 
You all saw some dirt! I dropped the AE Storage in the huge pile of dirt and can't find it im afraid :p
