What text editor are you using? If its Notepad++ check if theres any Red text. Last common mistake is that tab is used instead of spaces. YAML insists you use 4 spaces to replace a tab.

Someone on my server has used a Xycraft Fabricator to bypass NoItem crafting bans. Any idea how to prevent this ? Other than banning fabricators?

About the installation of mods into a already running server, you can just download the mods and put into the directory mods folder or the coremods folder in the server directory. About plugins, I suggest you obtain MCPC+. It will generate a plugins directory and you can place plugins in there.
