I don't like optifine due to how it tends to mess up most other mods (especially Tinkers), but I was having this same problem until I added Surge to the pack. Now I'm not having the problem.

To make the spawn in the center of the structure, you need to use the structure block in the center of your structure, and set the size manually instead of using structure corners. As for JSON, no clue. I gave up and used one of the preset islands.
