no, this has never happened to you. Vanilla minecraft durability mechanics say that 1 + 1 = 1.1 or whatever. Having the anvil add the death counts together would take dedicated coding to that effect. As this is clearly not the case then you are either mistaken, or lying.

except all you need is 5 shards with 1 kill each and you'll have a tier 5. It's not balanced with that bug.
