Can't reproduce with current description. Can't even open inventory after setting layout to CS. Other non-us qwerty layout(fi) works fine. Items can be moved into/from hotbar with shift+click. (Still not sure if this is the problem.) My recommendation: try vanilla mc, try newer vanilla...

Not 100%. FTB launcher asks confirmation for update. OP: Also server log (fml-server-latest.log) and full console log from client would make troubleshooting easier.

Do you know particular commit in lwjgl which fixes this problem? Does minecraft except shift + <number> or symbol which is created with shift + <number>? Does lwjgl 2 even support modifier keys?

Do you want us to guess or is it possbile to get all information asked in the the template? (troubleshooting is really hard without logs)
