I believe you have to place saplings in a 3x3 if its a 3x3 trunk to grow it. 2x2 if its a 2x2 trunk I think you get it for the rest.

I double that for Mouse Tweaks as well, I personally cannot play without Mouse Tweaks anymore.

Don't forget that you can force enchantments onto the tools using other mods. I have Efficiency 5 and Repair (Thaumcraft) on my Hammer (I didn't put moss on so I could get an extra modifier).
