I have a private FTB ultimate server for me and a few friends to play on. Everything that I can tell is fine with the server and I've had no complaints from anyone else except one player dies after logging into the server after a few seconds. There is no death message broadcast or that he can see when he dies and also the respawn button on the death screen doesn't work for him, he has to click the main menu button and rejoin the server but the same thing happens.

He joins, a few seconds pass, the death screen appears, he cannot press respawn and has to quit and re log etc etc

He used to be able to play fine and it suddenly started happening, nothing has changed as far as I know or can tell. Can someone help?

I can't say for sure, but the first thing I would do is change his spawn point to somewhere you know is safe. I forget the command exactly but if you cycle through the /help system by typing /help 1, then /help 2, then /help 3, and so on, until you see a command that looks like Set Spawn. You can set his spawn point without him logged in by nick. After doing this, have him attempt to login and see if that solves the problem.

If that doesn't work, have him go into his FTB folder and completely delete his Ultimate folder. Then when he attempts to play again, a fresh copy of the Ultimate client will be installed. Have him attempt to login again.

If it still doesn't work, then your best option is to delete his player information from the server manually. You can find this by going to your server folder, going into the folder that contains your world, and then going into the players folder. Find the file named after his Minecraft username, make a backup copy of it (and put it elsewhere), and then delete it - ONLY his file, of course. He'll lose anything he was holding on his person including armor and he'll lose any XP and any Thaumcraft research he's done. Honestly, given the choice between not playing and playing, you're going to go with playing. Just give him some stuff to compensate fgor what he lost. You can give him some xp with one of the server commands, I think it's /xp player amount or /xp amount player, but check the help menus to be sure.

I would be highly surprised if he was still having the same issue after going through all those steps if nobody else has a problem

Thank you very much we've sorted it. I'd never heard of this problem before and couldn't find any mention of anything similar anywhere so I was unsure of how to proceed
