Well, as I understand it, vanilla now has the option of being UHC-like, and real Mindcrack UHC with Etho/Doc/Pakk/Etc could potentially be ... more interesting with prayers.
Right now, the Kurt strategy of avoid all damage until the PvP is arguably superior -- but if the cost of the "risk" was lessened, if every day (20 minutes) people could recover 3.5 hearts, then you know that people will be at full, and getting enchantments and potions would be more important.
If they were to take that 2K overworld, divide by 8, and make the nether region a 256 by 256 containing only two or three fortresses, then suddenly being in the nether early becomes a serious advantage ...
