Recently, I have been thinking about creating my own modpack on MultiMc for myself and 1 or 2 friends. I knw, when making a pack, you need the mod author's permission to use it, and I assume in this case I still would. If we were the only ones to be playing it, without a server, would I still need permissions? Or if I was to be the only one playing it and no-one had any access?
 
Generally if the Mod is public then individuals have permission to use it. The permissions are important if you want to distribute the pack. Several mods I've seen people ask for on FtB aren't used because the mod developer doesn't want their mod distributed for one reason or another.

A good example is Fastcraft, which can help the frame-rate and lag issues with 1.7.10 packs... The developer wouldn't allow distribution because it was still a work in progress but you could go to the site and download it yourself. When the Dev was happy with it he released it for distribution and it's now probably in most 1.7 packs.

If you're only playing with a tiny handful of friends like that then it would be considered a private pack. Any reasonable author allows private packs, if not full permission altogether. As far as I'm concerned, do what you want. It's private, nobody knows the difference anyway. I've never asked permission to put together a private pack and frankly I don't intend to start.

Besides, if you're ever trying to use a mod which has severe restrictions on what you can do with it, then you should probably look for another mod. Historically those caused more trouble for users than they're worth.
 
If you are the only one playing then you don't need any kind of permissions. That'd be silly.
Sharing it with one or two friends, possibly also with a small whitelisted server, shouldn't be a problem either. I don't know anyone who would forbid that.
