(Tell you what, it was a close one, but not as close as the one that DID die)
Because they kept attacking the mooks instead of the Big Bad, leading to a prolonged fight that saw 3 of the party unconscious, one of whom died.

(nope it seemed to be some weird extradimensional pseudopotion that had effects determined by a dice roll - luckily a 6 seems to be resurrected with no ill effects!)
