Thanks for all the responses. I don't know what caused it but stopping and starting the server fixed it. It was weird, only lava and water placement were affected, everything else continued to work fine. I can only assume it was a memory leak with some strange consequences.

Let's try a different approach. If I had a freshly installed FTB Ultimate server and wanted to prevent people from placing water or lava, how would I do that?

I'm running FTB Ultimate on my server. I could have sworn that I could place water at one time, I haven't made any changes since installing the server, and I installed with all defaults. Any ideas where I could check to see if lava/water placement is disabled? Thanks!
