Not that I know of with the Hardcore wither mod enabled. The wither can literally teleport over and over again once you've killed it a few times. Its quite the fight to take it down now.

The answer is yes and i found the issue to be related to some of the things I asked above (hence I brought them up). I was able to locate the issue using OPIS.
