This solution worked. Not sure why this was triggered all of a sudden since the server has been up for a week or more, but alas, it worked.

I apologize, I did in fact say that. I realize now that the name I used in the console when opping was missing one character (one which I normally don't use on my nickname). I later went back and tried it again and it worked because I spelled my name correctly. I will try your solution -- thanks!
