Going to do this nice and form-like so I don't forget anything crucial. I get this problem both on a desktop and a laptop, the former running Windows 7 and the latter on 8. Both run on nVidia cards, a GT550Ti and a GT540M. I've ruled out Optifine by itself; the very same version of Optifine works flawlessly on vanilla Minecraft. (This does not absolve it from interaction problems, I realize.)

Detailed description of the issue / how can the issue be recreated:
I've only got theories at this point, but I can handily recreate the problem. After a while of playing on our server running the direwolf20 pack, I began to notice spikes that would lock up my framerate for just under a second. They occurred with regularity, and while it's possible to time some things around them, it's not easy.
A pattern started to emerge: Upon a clean start with the FTB launcher, things would be fine. Then as I went back to what I was doing, working in the Nether as opposed to Overworld, I'd start seeing the hang-ups again. I tried loading up my SSP map, and the problem persisted, and even got worse.
I can easily recreate the problem with Ages as well as with the Nether. Considering that the hang-ups grow more frequent and crippling upon a switch from server play to local play, I'm pretty sure my crackpot theory holds a little weight; that the game is simply not fully unloading dimensions, chunks, or entire worlds. Each switch from Age to Overworld to Nether to SSP map compounds the effect almost exponentially.
Texture packs and Optifine seem to have no impact whatsoever on the performance during these problems or otherwise. I play with several others on the server in question and none of them have the same issue. On that same note, I have recreated the problem on SSP maps, albiet slightly lesser. It takes visiting no less than three Ages/dimensions to get it from "annoying" to "unplayable", whereas it can happen in two or less jumps on the server.
