After a misadventure or two (chronicled here), I started carrying an emergency canvas supply bag with me all the time. A canvas bag has 27 slots. In it I carry:
  1. 10 obsidian, for a portal out of the Nether.
  2. A mystcraft book back to my base (I also have one on my hotbar, but a backup is incredibly important.)
  3. A stack of wood, for many purposes.
  4. A stack of charcoal for torches/fuel.
  5. A stack of flint, for arrows and in combination with iron for flint & steel to activate a portal.
  6. A stack of iron, for armor/tools.
  7. A stack of string, for bows/fishing poles/wool for a bed/bee scoops.
  8. A stack of books, for Mystcraft purposes.
  9. A bucket of water (1)
  10. A bucket of water (2)
  11. A stack of steak.
  12. A fire resistance potion (1)
  13. A fire resistance potion (2)
  14. A fire resistance potion (3)
  15. A remote orderer (logistics pipes.)
  16. An ender pouch (To input chest to my sorting system.)
  17. An ender pouch (To output chest for my remote orderer.)
  18. A minium stone (for multivarious purposes.)
  19. A silk touch enchanted pick.
  20. A silk touch enchanted pair of shears.
  21. A few blank soul shards.
  22. A blank maze/ore map.
  23. An electric Jetpack (backup).
  24. An Advanced Lappack (for recharging electric items with the MFSU.)
  25. An MFSU.
  26. An electric wrench (To pick up the MFSU again.)
  27. A prototype omniwrench.
With this, I can handle just about any problem, but I don't claim it's complete. What do you folks always keep with you?

I don't have a fancy list (of which seems EXTREMELY useful). I tend to bring the basics, and depending on where I die, I'll bring other things.

I tend to have an enchanted sword around at all times, even if it's weak, so I'll tend to bring that.
Having an extra portal gun, with one deployed at my base allows me to go back easily.
I have a Jetpack, a Lappack, and since I have a wither farm, I have an extra set of nano-armor.
I'll bring a bit of steak, I tend not to have clean cut stacks, so I'll take about 20, because 64's a lot.
Having a stack of dirt is nice, though wood is better because it is 4 stacks of planks that allow you to pillar up, and my treefarm will supply me with that.
I am usually too lazy to bring/find an axe in my base, so I use stone because I'll bring a pick.
Then I'll have an extra canvas bag. Enderpouches will be useful once I make one (though I have the materials, I haven't gotten around to it this map).
I've carried a redstone energy conduit and a machine or two in some cases, tesseracts would be better now.
And torches, never forget about torches.

Because I'm bad at battling/PvP/mobs, and the server I play on is hardmode (gamemode and gregtech), I tend to stick inside my house. When I die, it's usually from a fall...to my underground base. But, I have been spawn killed by zombie pigmen as a consequence for stupidly setting up a base 10 blocks away from one, I thought I could soul shard it (turns out you can't), and my left mouse button doesn't work all too well, and I haven't been able to stop my hitting/breaking of objects/mobs. I hit the zombie with a sickle, and it stole a sharpness diamond sword, so there wasn't much I could do.

You seem to have a lot of stuff, where do YOU die? o_O
*reads Mystcraft chronicles*

Carry on.

Anything and everything.

I have 5 RP bags on me right now.
I have one for RP stuff, Alloy wire, jacketed wire, tubes, covers, panels, screwdriver some of those wafers and any extra stuff from it.
I have one with BC stuff in it, engines, gears, gates, pipes. I also have all my extra TE stuff in here, energy conduits, liquiducts and any unused machines. I generally have some capsules of biomass and a few lava cells in there too.
One has any tools and weapons I pick up.
Any extra space and the other bags get random crap thrown in them.

The only thing I don't always carry with me are ores and ingots. Those get dropped off every time I go by my chest for that stuff.

Once I get around to working on bees and can make the upgraded forestry bags I will carry 1 of each of those also.

What are the silk touch shears for?
Cobwebs. It was in a recent update for vanilla.

Generally I just keep a bag with all my tools including forestry backpacks and such. If I carried some extra wires, pipes, and machines with me so I wouldn't have to run up to my chests all the time I would fall in lava. That's just my luck.

Why do you need cobwebs? Shears aren't something I'd bring anywhere if I die. Cobwebs aren't all that useful, if you're running from mobs just dig down. For wool, just bring a bed. There's little reason...unless you find the QUEST RAM.

To be fair, the silk touch shears are not really for emergencies, but I had the space and they're the sort of very infrequently used thing I'd forget to bring if I didn't keep it with me.

And mystcraft? :p
Bookshelves are 3 books each, for a storage price of 6 planks. If you need more books, consider using them.

Yeah, and Mystcraft. I copied the list out of my records of updates (since if the pack updates, all other things get wiped) and I hadn't put Mystrcraft on ther because there haven't been recent updates. Upping to bookshelves is a good idea, though the idea of a run of 64 worlds without one that had cows/sugarcane to restock is a bit overwhelming.
Did you have to change any configs for gravity gun and EE3?
Edit: and mystcraft and logisctics pipes

Not for Mystcraft or EE3, and I don't think GravityGun but I had to change the ID's for Logistics Pipes. I changed the sign ID to 1110 and the block id to 1111 and it worked just fine.

I don't use a bag so much for emergencies, as for avoiding "oh em gee I forgot to bring X with me again" kinda situations. So I tend to carry around all kinds of stuff I might want to use in many different situations.

Off the top of my head (Mindcrack pack, no Mystcraft, disabled portal gun):

- Always useful: Wood, extra torches, crafting tables, chests, levers, signs, redstone
- Tools: Omniwrench, screwdriver, silk touch pick, fortune pick, jackhammer, wand of equal trade, flint and steel, handsaw, several empty buckets
- Utilities: Goggles of revealing, spare jetpack, portable hole, enderbag connected to my sorting system
- Filler kit: Filler, a dozen or so landmarks, 10 glass, 10 bricks, redstone energy cell
- Fire resistance potions
- Two water buckets (can make an infinite pool anywhere)

I tend to bring a pocket crafting table on long journies, for the most part I tend not to need to craft anything, and since I don't have Mystcraft, finding trees isn't a problem so if I forget it (because I will), I'll be okay. When it gets updated, our server will add it, so I'm pretty pumped for that, mining ages will make everything a little bit easier, and a nice nexus void world will allow everyone to be closer.
