Summary of the problem TPS Lag and then Server Crash with this Crash Report

Pack Version 3.0.0

What is the bug? I run a server with 20+ online users at any given time. The server will be running fine for a while and then the TPS will drop and it will eventually crash with this bug report http://pastebin.com/5nBk4uDX

Is it repeatable? yes. Crashes ever 1-2 hours

Known Fix
 
Have you added any extra mods and/or does anything preface this occurring, like a player joining/leaving or some other action?
