Today on my server I noticed that my player was taking 3 milliseconds to update according to Opis. So I shoved my inventory into a chest and I went down to 80-90 nanoseconds. So I started putting my items back in my inventory one at a time, and as soon as one of my packs went in(I have a bottomless pouch from Kami and a Big black backpack from the backpack mod), I immediately jerked up to taking 1.2 milliseconds. And when I put in my second pack, I went all the way back up to 3 milliseconds. Just a PSA for server owners who may user lesser hardware.

Sounds like those are ticking inventories which IMO are just terrible about performance. Only things which are moving items like hopper should be ticking and even they shouldn't be ticking 100% of the time. Regular player inventories and chests should not tick unless called by a specific item like the RailCraft firestone.
