Server freeze then crash

it appears you have a corrupted potion launcher from TINKERS CONSTRUCT causing the crash. something to do with factorization and mobs

you could remove the mod and let forge clean your world then add it back, or use MCEdit to delete the effected chunk and let it regenerate
