Bump. Having the same issue, was hosting a server for a group of friends and myself. Left the room for 5 minutes, came back to my computer being reset(don't know what caused it). Once I ran the server again, I constantly get the same issue as OP.

Having the same issue, I can load the world in single player if I upgrade to SF2.5.2, but not 2.5 (which is what the server was) or the server.

Its a very rare issue but only known fixes are to update the mod, which means you'll need to update the pack to 2.5.2 to get the latest version

I managed to get /part/ of my server back for anyone having this issue.
One of the dimensions was causing the problem I guess so completely restarting a server and then moving over the r.0.-1.mca file from the region folder gave us back the main dimension, losing all nether and deep dark progress, as well as RFTools dimensions.
Updating to 2.5.2 helped as well.
