So after a lot of comfusion going around, I want to know how Optifine works with everyone. Does it increase your fps or does it kill it? It seems that everyone experiences it differently, so I'm interested in an average and if it could be recommended to use.

It used to work really well and boost my fps a ton but as time goes on forge has incorporated many fixes to vanilla that boost its efficiency making optifine all but useless for me.

For use with FTB (and certain other mod packs) it's not advised. Some of the mods have custom rendering code that Optifine can't do a thing about that's if they they don't cause conflicts like MPS does (or did).
So at best it wont effect a thing at the worst it corrupts your visuals. Not sure if it's ever crashed anybody though.

Optifine does increase my FPS. But I still don't use it due to how it effect mods. It is brilliant for vanilla. Highly recommend it, especially if you like to explore. But with mods it breaks more mods than it doesn't (at least on my and many others computers it does.) And most mods will have z-fighting and mods like TE will have tons of z-fighting if using Optifine. Z-fighting drives me bonkers. I'd rather deal with 5 fps than the normal z-fighting I get using optifine in a modded environment. :p

Is that when everything flickers, ProjRed lamps are the worst offenders if that's what you are talking about.

I would like to be specific. Optifine patched with karoynix's FML patch in conjunction with FpsPlus+ did help my FPS saga positively. As others have mentioned optifine may not be relevant amongst forge users starting 1.7 onwards. The FpsPlus+ mod is being ported for 1.7 and one that I eagerly wait for ;)

It used to be one of my favorite mods. Clear water, no void fog, better grass and those glorious mipmaps that made minecraft look 100 percent better. Even with many fancy options turned on, my game ran better with it than without. I waited and waited for mipmaps to be implemented in the 1.6.4 version but it never was, it was the reason I stuck with 1.4.7 as long as I did.

I also like optifine for the extra visual effects
Clear water
No Fog
Turn off smoke particles only (so i can fly around as a blaze and still see).

The extra FPS is a nice bonus, but not required as I can get 60fps+ without it.
