You're being abit vague there. If you mean keep the same world every time you make a new world, Lockdown. If you mean a map you can look at of the world, Journeymap.

Could you please elaborate a bit more? Creating a new world as in a new wold save or a new dimension. And what kind of map are we talking about? Gui map? Ingame item map like vanilla/twilight forest? And what do you mean with form on the new world?

You're still describing it a bit vaguely. Many mods automatically generate a specific item in the player's inventory when starting a new world, such as Tinkers' Construct or Hardcore Questing Mode.

No Problem, there is a mods list in the third party mod pack creative thread under Permissions. That may help in the future.
