Windows .exe Minecraft Crashing Very Often

My Mincecraft copy is crashing very often....but it is not minecraft crashing it is the java client it self. The error code is : EXCEPTION_ACCESS_VIOLATION (0xc0000005) at pc=0x000007fb41cf6220, pid=7016, tid=2144. The dump file is attached.
I am running YogCraft with optifine and an audio mod with the java parameters -Xms5120m -Xmx10240m.

It looks like it might not have enough memory. Try allocating more.

I'm allocating 10 gb out of 16 gb of memory, I'll allocate 12 gb.

That's WAY too much to allocate.
Can you post your crash log to pastebin.com as well as your clients console log in a separate paste?

I can't seem to find where my client's console log is save if it is, I will try and recreate it if I can. It is a sporadic occurrence.
