When starting a new map and starting IC/2exp, I build a very direwolf20 9x9, with a row of IC2 machines. My nuke goes right there. As a single block that can put out 120EU/t. In the room with all my machines and storage. It hasn't failed yet. And I don't see how it could.

I can totally buy the idea that a multiblock nuke, extending over a chunk boundary, could dynamically re-assemble as a smaller nuke missing some critical cooling components.
Yeah, for all intents and purposes, I agree. I have a habit of building my nukes away from my main building, but its just a habit. I've never had one actually detonate.

in ultimate I once made one giant room using warded blocks separated with 4 smaller rooms, guess what purpose they had:p

was only able to finish two of them and for some reason it looked like I had one running even when both of them where on. If I had one running it worked flawlessly, never figured out why, I probably just derped something

I have no idea how. I just know it can. A mod glitching is not an unusual occurrence by any stretch. However a reactor glitching has much more dire consequences than an item duct glitching. One will simply litter the area with items, the other will leave a big radioactive crater. Which brings me back to the original point of, if you choose to play with IC reactors, take the precaution of isolating it from your base.

So more than one nuke running caused a glitch? I was running 2 reactors at the same time when my base was cratered. I wonder if multiple IC reactors was the problem.

as far as I could see it only affected the outputted power, one was thorium the other a single quad cell of uranium
