How to diagnose connection lag?

You internet speed isnt relevant,
1. the ping to the MINECRAFT server is (ping to some speedtest website is also irrelevant)
2. as well as the server tick rate (should be 20).

Ping your minecraft server, and then check if the server is getting tick lag due to being overloaded by (probably) mass entities somewhere or bugged mods.

I pasted the results of the ping a few posts back, and TPS reports about 10ms for overworld and 2ms for nether. No other dimensions.

Yeah I was talking about everyone that started asking for/posting speedtest reports like it means anything in this context.

Your ISP might be doing something stupid with caching or the server itself is having major tick issues when someone logs in. Is it only you? Have you tried connecting through another system or having a friend that lives somewhere else with a different ISP connect into the world on your character to see if it is the same?

You might want to look into Jitter. Packets lost between your client and server can cause more lag than ping issues, especially if your server is located near you, which should give you a decent ping. Try redoing the same test on http://www.pingtest.net/ and post the results, that site will give you the jitter values.
