How do block and item textures work in 1.9.4?

Forgive me if this is what's considered a "noob question" - I've just started getting into modding.

In newer versions of Minecraft, the system for textures has been seemingly overhauled, moving to a JSON format. If someone could post a simple example of how to give an item a texture in 1.9.4, I would be very appreciative, as I have combed through tutorials and the Minecraft source code for several hours now and have found nothing.

Thank you! The first tutorial helped out greatly. I feel like this is the kind of thing that is only difficult once, and then becomes easy once you figure it out.
