Windows .exe Help me please!

so I've been playing DireWolf20's 1.6.4 modpack for a while now, and I've added a couple of mods that I like. These include; Metallurgy (added from the Horizons modpack), KeithsUtils, enchanting plus, More Enchantments!, and creyfish's furniture mod. I CAN play, but only for 2-15 minutes before i get a CTD. I'm not good with programming or things more technical than editing configs, so i dont have the means to sort this problem out myself, so i ask you for help.
