Download the latest nvidia driver for your graphics card, do a clean install (select advanced options and then check the box for clean install), restart your computer and try again.

If it still fails, post new log.

Figured as much. The Nvidia driver you had before was broken and did cause issues - not any of the issues in your log, so you probably didn't have Nvidia experience installed. Just wanted to be clear of that driver.

Try and update the JEI-mod to a later version (either the latest Release or one of the betas)

Post new log if it still fails.
