I have scoured the internet and all threads concerning this, and there appears to be quite a few people with the same problem. They sometimes list it different ways. It seems to be common among Nvidia users, though a friend on the server I play has a GTX 570 and is not having issue. Basically this is issue. I run at high fps most places but as soon as I enter Twilight, some Mystcraft ages, and or the Jungle Biomes while in flight it starts to degrade with "hitches" which become spikes....which becomes 1-20 seconds of freezing. It appears to happen around complex leafy tree areas.
I am running on an i7 3770k with GTX 670, and 8GB of ram.
I have upgraded to latest nvidia drivers.
I have uninstalled and reinstalled java (win 7 64) (no prev versions)
I have uninstalled and reinstalled FTB. I have made sure Javaw.exe is listed in Nvidia applications.
I am NOT running optifine, and not likely too do to it's issues with some mods.
I have played with memory settings from 2gb to 5gb not much difference.
When the problem Occurs disconnecting and reconnecting does not stop issue, only completely closing down minecraft and restarting it does, but it soon comes back.
I have cleared other processes to best of my ability, I have minimized any effect Anti-Virus could have.
What I have found is this seems to be a common problem for many people, but I never had it on single player. (but I didn't use 1.5.2, I used 1.6.4 and 1.7.2) and other people on this server do not have issue just two of us. I am in TS with other players and server admin when playing and they are not getting any issues. I have a 100+ Mbs connection with server (fiber) so not an issue.
