Hi there.

I'm using the FTB Ultimate Pack.

I did a complete re-install of everything. Clean server files and a clean client, but I still get the "ID Mismatch" issue...

How do I fix this? And how can I make sure that players who join the server won't get the same message? Is it server-side or client-side?

This is quite a common issue. Copy your configs folder from the server to your computer and replace what is already there. This should fix it, if not then let us know.
