Iron ingot on top
Division Sigil on in the middle
Diamond on the bottom.

The Division Sigil is a random chest loot item.

Also also note that you have to use the ingots immediately after crafting them and do it quickly, if you take too long to craft whatever you want to craft with them or exit the crafting grid they'll explode.
