Compiling a list of all item/block IDs

I'd like to build a map from item IDs to item names, for all item IDs I have in my version of minecraft. I have no idea how hard this would be, nor where to find info on where and how minecraft stores this info, but I figured mod makers would be able to give clues. Please excuse me if this is the wrong place for this question. Thanks!

In case you need to know for custom packs, there is an option in nei to generate this type of file as well as a few others.
