Windows .exe Bug - premature tool breakage due to durability bug

When I make any type of tool (axe, pick, shovel, etc) even though it says the durability is near full, after two or three uses, the tool breaks. Durability is still nearly full when the tool breaks. No reason for a new diamond or iron pick to break after two uses on stone, or an iron shovel to break after two blocks of sand. The FTB server was just downloaded off the site, and the client launcher is V1.2.7. Server and client restarts don't fix the issue. There was no problem initially, but after about 4 hours of building, the problem started and is permanent now.

O/S is Win7, 16 GB ram, 64 bit Java (client/launcher crashes constantly with 32 bit Java), nothing else running on computer except for the latest FTB server and the FTB client.

Ok, so no answers, and even after a reinstall of both the server and client, it still happens. Are inventory tools supposed to take damage when a mob hits you? This is the only thing I can think of that is happening that would be causing the damage over time of the inventory tools.
