It seems you need a dynamic dns service, here are some free ones:

• http://www.noip.com/
• http://dnsdynamic.org/
• https://freedns.afraid.org/
• http://www.twodns.de/

You set these up so that a (sub-)domain always points at your home IP, then you let people connect to that.
