What is the bug:
It is possible to exploit EE3 in combination with Mekanism and optionally Simple Condensers in order to generate infinite EMC (and thus infinite items). Simple Condensers lets the exploit be automated, otherwise it is not required as the transmutation tablet can be used for the same purpose.
The crux of the issue is that Mekanism introduces a few recipes that have a net EMC gain at the expense of a small amount of power draw. The most potent I've found is crushing flint into gunpowder, however others exist as well (getting additional planks/sticks out of logs, and smelting rotten flesh into leather). The sticks one is fairly minimal but flint -> gunpowder gives a net gain of 188 EMC/operation and rotten flesh -> leather gives a net gain of 40 EE/operation.

Setup works like so:
Condenser producing Flint gets input into crusher, output loops back into that condenser and then into a second condenser producing whatever you want it to produce (let's say Diamond Blocks for the sake of explanation). After filling the first condenser, excess gunpowder from the crusher gets fed into the second which accumulates into infinite Diamond Blocks over time. If scaled to 7 Elite Crushing Factories along with using Minium Condensers, this produces 1 Diamond Block every 54 seconds.
The condensers can be replaced by you manually picking up gunpowder, tossing it in the transmutation tablet, and getting replacement flint for the crushers; as such the condensers are not strictly necessary.
Known Fix:
Disabling the exploitable recipes is likely the best way of going about it; other mods in the modpack are more than capable of making up for the lack of leather, gunpowder, and sticks (soul shards spawners can provide an infinite source of leather/gunpowder, and regular minecraft crafting combined with progressive automation can yield an effective tree farm giving you all the sticks you need without requiring enhanced yield recipes).
