Check the openblocks inventory restore funtionality, if you can't find a gravestone. '/ob_inventory restore'.

The 'openblocks' mod saves player inventory on death. Google '/ob_inventory restore' for more info on how to use this.

Mystcraft profiling runs only once, afaik. This sounds more like garbage collection issues, so either too much, or too little memory. Try somewhere between 2 and 4 gigs.

Do you still have a problem if you remove that additional mod?

Well, you could post the normal logfile (see the log directory), but since this turned into a jvm crash, I have no idea how to continue. I do think it is related to graphics/opengl, so maybe play with the graphics settings, see if the crash goes away?

I don't know. Maybe it is the same problem, maybe it is new. You could try and verify that using the latest FML version, but I think you'll have to update a lot of mods then as well. I would definitely report it to the FML people on github.

Can you try this again without optifine, and upload that logfile?

The log directory (not crashlogs) should contain the normal logfiles.
