Recently bought an SSD so I've had to re-install a lot of stuff, one being FTB. I only put OF into FTB about 2 weeks ago on my old install so I had a good idea of how to do it. I placed OF (OptiFine_1.4.6_HD_U_D5) into the instmods folder and it appeared in the edit mod pack screen as it should. I started MC to see the line stating OF was on in the bottom left along with forge/MCP/other stuff.

When I added sphax 128 (base pack and FTB addon into one of course) I got this as a result:

I'd embed but it's a somewhat large image.

Try adding Optifine in using the launcher.
Go to Edit Mod Pack... and add it in the JarMods tab.

Also, does that happen with any texture pack or just that one?

Random other thought, you aren't installing a 1.4.6 mod on a 1.4.7 mod pack, are you?

I added OF by putting it in a random folder and browsing for it, as well as chucking it in the folder with the forge zip so it auto appeared.

I don't have any other TP's. But I don't think it happened with default.

Yes it's 1.4.6 OF on 1.4.7 MC, the version after 1.4.6 is 1.5.1
