I'm on the horizons pack. Is there a setting I have to flip to get them to generate? Eight people are on my server and we've been playing about 3 weeks and have never seen one.

You need to go on the server Roguelike config and set "doNaturalSpawn" to true. It's on false by default for some odd FTB decision ...
