Problem Portforwarding is only temporary

Hi, I run an FTB direwolf server for my family and friends(no more than 4 at a time) and unfortunately every time one of them that doesn't live in my house tries to join it won't work. I then redo the port forwarding, and it works, but the next day(or sometimes a few hours passes) and they can't connect again. The PC running the server is 192.168.0.21 has not changed since day one. I tried making the server a DMZ but that again only worked temporarily. I have an arris tg862 modem\router

Um, the problem is the clients aren't connecting to 192.168.0.21, they are connecting to your external IP. Your ISP needs to give you a static IP for that work.
