Optfine and dissappearing textures

I installed optfine by putting the .zip file in the instmods folder of ftbbeta. When i logged in my server it was fine but over time textures dissapeared. Chests dissappeared but were still there and Cows suddenly lost there heads, along with all my other animals. How do i fix this while still using optfine?

Try another Optifine. This problem happen to me once worked out that i had the wrong one.

it seems to be working with the hd b2 standard version. does it not work with the ultra version?

ultra version for 1.4.2 is known for bugs.

I stay completely away from ultra version for opifine.
