FTB Revelation keeps crashing after teleporting

Hi,

so im playing FTB Revelations 3.3.0 an every so often (almost every day) the game just frezes after starting to load the world after teleporting (normaly to my base but not always). I have added a few extra mods (nuclearcraft and optifine). If anyone knows whats causing it I would appriciate it.
