Problem FTB revelation crashes every single time after teleporting

so it was fine at first, maybe a crash once a while, but it just keeps getting worse and worse to the point where i can't play it anymore. It crashes every single time i teleport (doesn't matter where to). Im not ready to quit this world yet so any help would be apreciated. crash report down below. :D

ps. already tried disabling config options for p455word, it did nothing. and i have optifine instaled.

Log indicates render issue, remove optifine and try again.
