...or maybe I just never noticed it before.

The two middle lines, or ones similar, are displaying on every item when using NEI. Did I hit something by accident? If so, how do I turn it off? I think that I've tried just about everything under NEI > Options.

Did you hit F3+H at some point? That turns on Vanilla's debug mods, which is the first thing that comes to mind that could cause that.
 
One of those lines if from vanilla and the other one is from NEI Integration. If you don't want them to be doubled up like that you can disable the other one in neiintegration.cfg. D3Core also has some of those if you have it installed. I think vanilla didn't have that info at first but now the mods are just duplicating a vanilla feature.
