Veinmining a 3 connected Ender IO ME Conduits, causes the pick your using to instantly break.

I've noticed this as well in 2.5.2, what is happening is you're still vein mining the first conduit after you've broken it which will take away all your hunger (if you pick lasts long enough) as well as break the pick. I've also noticed that this could be abused to spawn slimes as well. Example shown in this gif here https://gfycat.com/FreeSpeedyIncatern
