What is the bug:
After launching the server it randomly has a chance to crash after some time. It does always eventually seem to crash. This generally happens ever 1-30 seconds. It seems to happen when someone is join but not always. Multiple people joining seems to make it worse.
Mod & Version:
Maybe chisel or chicken chunks and they would be the version that is in direwolf20's 1.2.1
