What is the bug:
Shift-clicking on Witchery Ritual Chalk will cause it to stack with any other chalk in the container, regardless of damage. All stacked chalk will take on the damage value of the chalk in the container.

Place damaged (used) Ritual Chalk in a chest.
Shift-click more Ritual Chalk from your inventory.
The chalk from your inventory will stack with the damaged chalk in the chest, effectively damaging it until it matches the chalk in the chest.
